Adoption Spay/Neuter & Pickup Contract

Required for every adoption. This contract covers scheduling your dog's spay or neuter, paying in full before surgery, and same-day pickup with photo ID.

We schedule the surgery

Once your adoption is approved, we book your dog's spay or neuter appointment with our veterinary clinic and notify you of the date, time, and location. You do not need to book it yourself.

Payment is due before surgery

The adoption fee and surgery fee are one combined amount, paid in full to Trouble Off Your Hands K9 before the appointment. Payment in advance is what confirms and holds your dog's surgical slot.

Same-day pickup only

Your dog is released the same day as the surgery. Pick up at the clinic, or arrange a public meet up with us — either way, pickup must happen that same day. Overnight boarding at the clinic is not available.

Bring photo ID

Photo identification is required at pickup. The ID must match the name on this contract. No dog is released without it.

Read, Agree & Sign

Please complete every section below. You will be asked to sign electronically, and you will need to bring photo ID on pickup day.

Dog & Adopter Information

Photo ID Verification

We are required to verify the identity of every adopter before a dog is released. Upload a clear photo of your government-issued photo ID (driver's license, state ID, passport, or military ID). Both sides are required. The name on the ID must match the full legal name signed on this contract.

Stored only to verify your identity

Optional, but helps us verify you faster

How we use your ID: Your ID is used only to confirm that you are the person named on this contract before your dog is released to you. It is stored securely, is not shared publicly, is not used for marketing, and is deleted once your adoption is complete. You must still bring your physical photo ID to pickup — the upload does not replace in-person verification.

Spay/Neuter & Pickup Agreement

Please read the terms of this agreement carefully. By submitting this contract you agree to all of the following:

Spay / Neuter Scheduling

  • Trouble Off Your Hands K9 will schedule the spay or neuter procedure for the adopted dog named above with our veterinary clinic. We will notify you of the appointment date, time, and location.
  • If the dog has not already been spayed or neutered at the time of adoption, the dog remains under our program until the procedure is completed, unless separate written arrangements are made.
  • You agree to make your dog available for the scheduled appointment, and to follow all pre-surgical instructions we provide, including withholding food and water as directed.

Payment Terms — Due in Full Before Surgery

  • The adoption fee and the surgery fee are one combined amount, quoted to you by Trouble Off Your Hands K9. There is no separate charge for the spay or neuter procedure.
  • That combined fee is due in full, in advance, before the surgery takes place. Your dog's surgical slot is not confirmed and will not be held until the fee has been paid in full.
  • If payment in full is not received by the deadline we provide, the appointment will be cancelled or rescheduled without further notice, and that date may be offered to another client.
  • Amounts already paid toward the fee will be applied to the cost of the adoption and the surgery and are not refundable as cash except as required by law.

Surgical Risk Acknowledgement & No Refund

  • I understand that spay and neuter procedures carry inherent surgical risk, including the risk of complications or death.
  • If the dog does not survive the procedure, or dies during or as a direct result of the surgery or anesthesia, no refund of the adoption fee, surgery fee, or any other amount will be owed or paid.
  • I understand that any surgical or aftercare costs arising from complications are my responsibility, regardless of the outcome of the procedure.
  • I confirm that I am the person authorized to consent to veterinary treatment for this dog, and I accept that authority and responsibility.

Pickup Terms — Same Day Required

  • Same-day pickup is required. The dog must be picked up on the day of the scheduled procedure — the same day as the surgery itself.
  • You may pick your dog up directly from the clinic, or arrange to meet up with us at a public location. Either option must take place on the same day as the procedure.
  • The dog cannot stay overnight at the clinic or with us unless a separate arrangement has been agreed to in advance in writing. Additional boarding or handling costs may apply to any arrangement we agree to.
  • If able, bring a crate, carrier, or restraint to pickup, and keep your dog quiet and limited to leash-only activity for the ride home and during the recovery period.

First Right of Refusal & Return of the Dog

  • Trouble Off Your Hands K9 holds the first right of refusal on this dog. If the adopter can no longer keep the dog, for any reason and at any time, the dog comes back to us.
  • The adopter agrees not to sell, resell, trade, give away, or transfer this dog to any third party — including a friend, family member, breeder, trainer, or another organization — without our prior written consent.
  • All return costs are the adopter's responsibility. The adopter agrees to pay all costs associated with getting the dog back to Trouble Off Your Hands K9 — including but not limited to transport, travel, shipping, boarding, veterinary care, and any other cost of returning the dog. Trouble Off Your Hands K9 is not responsible for any portion of the cost of returning the dog.
  • This applies whether the dog is returned at the adopter's request or at our request, and it applies regardless of how far away the adopter lives — including adoptions out of state. If the adopter lives outside the state, the adopter remains responsible for arranging and paying for the dog's return to us.
  • The adopter agrees to return the dog promptly when asked. Failure to return the dog may result in our pursuing all available legal remedies.

Photo ID Required at Pickup

  • You must present a valid, government-issued photo ID at pickup.
  • The name on the ID must match the full name signed on this contract.
  • If someone other than you will be picking up the dog, that person must present their own photo ID, and you must provide written authorization naming them in advance. We will not release the dog otherwise.
  • No dog will be released without proper identification. There are no exceptions to this.

Aftercare

  • Keep the surgical site clean and dry, and prevent your dog from licking or chewing at the incision. Use the cone or recovery suit provided or recommended by the clinic.
  • Give all medications exactly as prescribed, and follow all post operative care instructions from the clinic or from us.
  • Call the clinic or contact us immediately if you notice bleeding, discharge, swelling, lethargy, vomiting, or any other concerning symptom after surgery.

Acknowledgement & Electronic Signature

Picking Up at the Clinic or a Public Meet Up

Every pickup happens on the same day as the procedure. You can collect your dog directly at the clinic once they are cleared for release, or we can arrange to meet you at an agreed public location. Either way, bring your photo ID and be ready to take your dog home the same day.

We will confirm the exact pickup time, location, and any clinic release instructions with you once the surgery is scheduled. If your plans change, contact us right away — a dog that is not picked up on the scheduled day may incur additional handling or boarding costs.
