What This Form Does
Submitting this form or expressing an intent to pay does not, by itself, constitute completion of an adoption, transfer of ownership, or reservation of a dog, unless otherwise confirmed in writing by Trouble Off Your Hands K9. Payment may also be subject to a separate adoption agreement, purchase agreement, training agreement, invoice, deposit policy, or other applicable terms. This form documents a stated intent to pay and is not proof that payment has been received — payment is considered received only after it has been successfully processed, delivered, or otherwise confirmed by Trouble Off Your Hands K9.
Reservation & Priority
A deposit or early payment may be required to reserve a dog or a service slot, and making such a payment secures availability on a first-come, first-served basis until the Intended Payment Date identified on this form. If the full payment is not completed by that date, the client may lose priority, reservation, or availability status without further notice, and the dog or service may be offered to other adopters or clients.
Electronic Signature & Contact Information
The client's typed name and date on this form constitute an electronic signature with the same legal force and effect as a handwritten signature, including entry into a valid and enforceable agreement. The client confirms that the name, phone number, email address, and billing address provided are accurate and current, and agrees to update Trouble Off Your Hands K9 promptly if any of them change before full payment is completed.
Late Payment & Fee Terms
Unless otherwise agreed in writing, payment is due within two (2) weeks (14 days) of the Intended Payment Date identified above. If payment in full is not received within that 14-day period, a late fee of $25.00 may be added to the outstanding balance, plus any additional charges stated in the applicable adoption, purchase, training, or deposit agreement. A payment that is more than 60 days past due may also affect the reservation or availability of any dog identified above, and the dog may be offered to other adopters or purchasers.
Deposit & Cancellation Policy
A deposit or partial payment made to reserve a dog or a service slot is non-refundable if the arrangement does not proceed as a result of the client's own action or inaction. This includes, but is not limited to: failure to pay the remaining balance by the due date, cancellation of the training program or boarding dates, failure to complete the adoption or pickup, or otherwise not following through on the Intent to Pay documented on this form. In these cases the deposit or partial payment made may be retained by Trouble Off Your Hands K9 as a cancellation or reservation fee, at the sole discretion of Trouble Off Your Hands K9. The reserved dog or service slot may then be offered to other clients.
Failed / Returned Payments
If any payment the client authorizes fails or is returned for any reason — including insufficient funds, a closed or frozen account, a reversed or disputed electronic transfer, or a declined or failed electronic payment — the client remains responsible for the full outstanding balance. A returned-payment processing fee of up to $35.00 may be added to cover the costs of handling the failed payment.
Accuracy of Information
The client certifies that the information provided on this form is accurate and reflects the payment they intend to make. Providing false, incomplete, or misleading information on this form — including falsely stating that a payment has been made when it has not — may constitute a material misrepresentation and may void any reservation or availability established by the form, and the client may remain liable for any amounts properly owed. Trouble Off Your Hands K9 reserves the right to verify any information provided on this form.
Non-Refundable Deposits & No Chargebacks
Except as otherwise required by law, deposits and partial payments made to reserve a dog or service are non-refundable once a reservation is confirmed. A reservation is deemed confirmed when Trouble Off Your Hands K9 receives the deposit or partial payment and communicates confirmation to the client. If payment is collected in error or for an amount the client wishes to dispute, the client is expected to contact Trouble Off Your Hands K9 to resolve the matter directly rather than initiating a dispute, reversal, or chargeback of the payment without first notifying Trouble Off Your Hands K9 and allowing a reasonable opportunity to correct it.
Collection & Recovery
If a payment remains outstanding beyond its due date and the default is not cured within the applicable notice period, Trouble Off Your Hands K9 may exercise its available rights and remedies, which may include referring the matter to a collection agency or seeking recovery through lawful means. The client is responsible for the outstanding balance, any applicable fees set out on this form, and any reasonable collection costs permitted by law.

By submitting this form and signing below, the client acknowledges they have read and understood the Late Payment & Fee Terms, Deposit & Cancellation Policy, Failed / Returned Payments, Non-Refundable Deposits & No Chargebacks, and Collection & Recovery terms set out above.
