Required Before We Take Your Dog

We cannot accept your dog without both of these. Your dog must be current on vaccinations, and we need the records and a copy of your photo ID on file before care begins.

Vaccination Records *

Upload a photo or scan of your dog's current vaccination records (rabies, distemper/parvo, bordetella, and any others your vet provides).

Copy of Government-Issued Photo ID *

Upload a clear photo of your government-issued photo ID (driver's license, state ID, or passport). The name must match the owner name on this form.

If records are not provided, we may refuse your dog. Trouble Off Your Hands K9 reserves the right to refuse service to any dog that is not current on vaccinations or whose records cannot be produced. If your dog is refused for this reason, any fee paid is non-refundable.

Payment is arranged directly with us and is not collected through this form. By signing, you authorize payment of the rate stated above, and you understand that a last minute booking fee may apply. Payment is due before or at the start of the sitting period unless we agree otherwise in writing.

Liability Terms

These terms exist because last minute bookings leave us no opportunity to evaluate your dog beforehand. Please read each one.

No liability for illness or injury. Trouble Off Your Hands K9 is not liable for any illness, injury, death, escape, or loss of your dog during or after the sitting, including any pre-existing or undisclosed condition, unless caused by our gross negligence.
No liability in your home. If sitting occurs in your home, we are not liable for damage to your property, items lost or stolen, or worn surfaces, and we are not responsible for pre-existing issues in the home.
Undisclosed conditions are your responsibility. You are responsible for fully disclosing every known medical and behavioral condition. We are not liable for anything arising from a condition you did not disclose.
Veterinary costs are yours. If your dog needs emergency care, we may seek it, but you are solely responsible for all veterinary costs. We are not obligated to pay any portion.
We may refuse or end a sitting. We may refuse your dog, or end the sitting early, if your dog is not current on vaccinations, if records are not produced, if a disclosed or undisclosed condition makes care unsafe, or if your dog poses a risk to people or other animals.
No guarantee of availability. This form is a request only. No agreement exists until Trouble Off Your Hands K9 confirms the booking with you directly.

I understand that undisclosed conditions are entirely my responsibility and that they are not liable for anything resulting from a condition I failed to disclose.Emergency Veterinary Authorization: * If my dog requires emergency veterinary care, I authorize Trouble Off Your Hands K9 to seek that care, and I agree that I am solely responsible for all veterinary costs. They are not obligated to pay any portion of it.No Guarantee of Availability: * I understand that submitting this form is a request only. It does not guarantee that Trouble Off Your Hands K9 can care for my dog, and no agreement exists until they confirm it with me.
